Community health needs assessments

Development of five health and wellness plans

MNP has years of experience in successfully delivering projects to Indigenous health organizations. Below are examples of completed projects demonstrating MNP’s ability to provide insights and advice to help health organizations optimize service delivery and improve outcomes from Indigenous health programs.

MNP helped a client who was entering a block contribution funding agreement by developing a community health plan. This resulted in the client securing a five-year funding agreement.

MNP successfully assisted a health department in achieving accreditation by facilitating policy development, strategic planning, and staff training, enabling them to progress toward their goal of incorporation.

Substance Use and Addictions Program (SUAP)
The SUAP is a new federal funding opportunity that guides the federal government’s work to save lives and minimize substance-related harms for people who use drugs and alcohol, their families, and communities.
First Nations, Métis, and Inuit not-for-profit organizations and Indigenous governments can access funding for the four following funding priorities:
- Supporting and enhancing the role of People with Lived and Living Experience (PWLLE) within organizations that address substance use across the comprehensive continuum of care.
- Services and programs that target post-treatment aftercare and transition back into the community.
- Addressing alcohol abuse disorder
- Addressing adult tobacco cessation
This funding opportunity closes on November 22, 2023, at 2:00 p.m. EST.
Health Services Integration Fund
The Health Services Integration Fund (HSIF) is an initiative supporting collaborative planning and multi-year projects aimed at better meeting the healthcare needs of First Nations and Inuit.
HSIF funding can be used to:
- Implement and test integration agreements and arrangements.
- Develop new governance models to increase First Nations and Inuit control over health service delivery.
- Support multilateral tables with representatives from First Nations and Inuit organizations, provincial and territorial departments, and Indigenous Services Canada to work together to support integration, aggregation, and work on joint initiatives.
